AED Application Form (Philips)

Application form to complete if your facility is interested in applying to receive a (recommended) Philips Automated External Defibrillator donation from Kick Start My Heart. AED placement decisions will be based on population and probability (i.e. how many people use the facility and how probable is it that a cardiac emergency event will occur in this population). Senator Nancy Spence - YBSAD Support Letter

Letter of support, signed by Colorado State Senator Nancy Spence, regarding the Youth Battling Sudden Arrhythmia Deaths (YBSAD or Why Be Sad) project, aimed to promote, fund and facilitate youth heart screening programs and AED placement throughout the state of Colorado. The project team comprises Brittan Sutphin, Kick Start My Heart, the Daniel J. Linger Memorial Fund and the Anthony Bates Foundation.
