AED Application Form

Official application form to complete if your facility is interested in applying to receive a Automated External Defibrillator donation from Kick Start My Heart. AED placement decisions will be based on population (i.e. how many people use the facility) and probability (how probable is it that a cardiac emergency event will occur in this population). One page document providing a summary of how everyone can contribute to this worthy cause. Includes ideas such as: registering a soccer team to play in the event; playing as an individual soccer player; raising sponsorship from your own participation; finding a sponsor who can raise money; donating merchandise or a gift-in-kind to the silent auction; sponsoring an AED and training program for a school or group; or volunteering to help out at the event.
